Coaching Class Admission Form Format (Word/PDF): Free 2027 Template for India
What should a coaching class admission form contain? At minimum: student identity, parent or guardian contact, class/course, chosen batch, agreed fee plan, payment record, communication consent, signatures, and an office-use box. The form should answer one operational question six months later: what exactly did this family join and agree to?
This is a practical coaching class admission form format for Word or PDF, written for Indian tuition centres and small coaching institutes. Copy it into Word or Google Docs, replace the bracketed fields, add your logo, and export as PDF. It is not legal advice; state rules and the type/size of your centre can change what you must disclose.

The fee section most forms get wrong
Do not write only “Course fee ₹24,000”. That leaves four unanswered questions: when is it due, can it be paid in parts, what has already been paid, and what happens if the student exits? Record the total agreed fee, payment schedule, first receipt number and remaining balance. If the fee changes by batch or subject, the chosen batch must sit next to the fee—not on another loose page.
After admission, create the same student in one fee ledger. A signed form in a file and monthly UPI screenshots in WhatsApp are two disconnected systems. For the monthly workflow, use the coaching-centre fee collection guide.
Admission form vs enquiry form
| Enquiry form | Admission form |
|---|---|
| Before the family decides | After seat/course is agreed |
| Name, phone, class sought, source | Full student, guardian, batch and fee record |
| Under two minutes | Completed carefully with signatures |
| Follow-up status | Student ID + first receipt |
Need the first-stage sheet? The free coaching register templates include an admission enquiry log. Once the parent confirms, use the fuller format above.
Fields to collect carefully—or skip
- Use a primary WhatsApp number and an alternate contact. A mistyped number breaks every future reminder.
- Make Aadhaar optional unless a specific lawful requirement applies. A neighbourhood tuition centre usually does not need an identity number merely to teach a batch.
- Do not pre-tick marketing consent. Class updates and promotional messages are different purposes.
- Avoid vague photo consent. If you want to publish a result or testimonial, ask specifically at the appropriate time.
- Do not promise rank or selection. Record the course being provided, not a guaranteed outcome.
The same-day admission workflow
- Assign the student ID and exact batch name.
- Write the agreed fee schedule before accepting money.
- Record the first cash/UPI payment and issue a receipt.
- Add the student to your digital fee list the same day.
- Send one WhatsApp confirmation: batch, start date, timing, next due date.
- File the signed form by student ID—not by a nickname.
If parents discover batches online, connect this back-office form to the public batch registration and paid-demo workflow. Enquiry is the front door; admission form and fee ledger are the record.
After the form, create the working record
